21834 and TS, since the title is not in itself a sentence, it could be either, depending on what's assumed to complete the sentence, e.g.:
(This is) my ex-girlfriend and I.
(This is a picture of) my ex-girlfriend and me.
At least this debate about grammar serves to distract the more sensitive among us fom the picture's rather distressing subject matter.
